You may schedule a screening appointment with us through our website by by providing certain personal or contact information (e.g., email address, phone number, gender, date of birth). You may also schedule a screening appointment with us through our Customer Care Center by providing the same personal or contact information to one of our customer service representatives.
If you choose a heart screening or other service through our website, we may ask you to provide us with personal information and/or health information relating to you and any family member. This information will be used as a component of the overall screening assessment by licensed cardiologists and physicians that are part of the CAS Foundation Business Partner network.
If you choose to obtain heart screening services via our site, you must provide the following information: your name, telephone number (in case our customer service personnel need to reach you about your apoointment), mailing and/or shipping address, email address (so that we may send an email message about your appointment and, if you opted-in, to send you our email newsletter and/or special offers). All information, once on our server, is protected from any external intrusions.
