Sign up for a Screening

Before You Start ...

A few things you should know to ensure a successful registration:

NOTE: SafeBeat Initiative heart screenings are for the students that attend the respective high schools only. These are not public screenings.

You will be asked to select the event location, date, time, and type of screening program. The following screening programs may be offered at your desired event:

• HeartScreen: Includes personal and family history cardiac questionnaire, body mass index (BMI) calculation, blood pressure reading, and an electrocardiogram (ECG).

You will be asked to create a username and password to register. You will be required to login with your username and password to schedule a heart screening appointment for other family members, as well as to access your results online.

You will also be asked to provide personal information about yourself and/or your child, including:

• Name
• Mailing address (to receive results)
• Phone number
• Email address (to receive appointment and payment confirmations)
• Date of birth
• Height and weight (for BMI calculation)
• Gender
• Ethnicity
• Personal and family history cardiac questionnaire (for the person being screened)

Finally, you will also be asked to provide consent for the heart screening, and to make a voluntary contribution.

You should have all of the above information available when you start the registration process. You will not be able to save your answers until consent is provided. If you are unable to complete the registration process, you will have to start the registration process from the beginning.
